Preparing for a visit
In order to better serve you, please take the time
to prepare for your appointment and bring the necessary information
with you to your appointment. This will enable us to create an accurate
and up-to-date medical file, which will assist our specialists in
providing you with the best possible care. Feel free to write down
any questions you may have beforehand, so that the doctor can address
these during you visit.
Items you should bring with you:
- X-rays, CT scans,
and/or lab results: Recent x-rays, CT scans
(the actual films or a CD with the pictures on it,
along with a copy of the report), and labs, which will
assist our specialists in gathering the necessary information
needed for a complete medical review.
- Current medications in their original containers
any vitamins, over-the-counter medications or herbal preparations
information and referrals
Including current insurance card, workers' compensation or auto accident
claims information, including the claim number and name and address
of where to send the claims.
- Forms: worker’s
compensation, disability or any other forms that need to be filled
out. Some forms require a thorough review of the medical record
before they can be completed, therefore we may assess a nominal
fee that is to be paid in advance when the form is left with us.
